Job Overview

Verovian Recruitment Agency is seeking a compassionate, dedicated, and person-centred Social Care Officer to join a team in Tyne and Wear. This role involves supporting vulnerable adults in the community and promoting their independence, dignity, and wellbeing.

You will play a key part in assessing needs, developing support plans, and working collaboratively with other professionals, service users, and their families to provide holistic and effective care.

Job Specifications

• Conduct care assessments and support planning for service users in line with statutory frameworks.
• Monitor and review individual care plans, ensuring ongoing suitability and effectiveness.
• Liaise with external professionals and agencies to coordinate and deliver comprehensive care packages.
• Promote personalisation, independence, and choice through care planning and delivery.
• Keep accurate and up-to-date case records and documentation in compliance with local authority standards.
• Safeguard individuals by following relevant policies and procedures.
• Support discharge planning from hospitals and transitions between care settings when required.

Candidate Requirements

• Experience working within social care, ideally supporting adults or older people.
• Enhanced DBS on the Update Service
• Two Satisfactory References
• NVQ Level 3 Health and Social Care or Equivalent
• Strong written and verbal communication skills.
